5. Marking components noted in Proforma‑II:
BAMS academic marks as per Advertisement Clause 5(i)(ka); MD/MS academic marks as per Clause 5(i)(kha) @ 10 marks; contract teaching experience @ 02 marks per year (maximum 10); journals/publications marks (maximum 04).

13. Proforma-I:
Duly filled and signed Proforma‑I for document verification, completed in blue/black pen and submitted with supporting photocopies as indicated on the form.
14. Teacher Code/NTET proofs:
Attach NCISM Teacher Profile and/or email regarding generation of Teacher Code, or NTET score card/eligibility certificate showing issue date and validity.
15. Proforma‑II (this document):
Completed and signed Proforma‑II capturing BAMS and MD/MS academic marks, contract teaching experience and journals/publications details.
16. Experience proof details in Proforma‑II:
Provide experience certificate number and issue date, appointment letter and issue date, and indicate availability of salary slip (Yes/No).
17. Publication details in Proforma‑II:
For each publication mention title, type of journal, whether main author, name of journal, indexing agency as per advertisement, Serial no. & year of UGC‑CARE/SCI/Scopus/PubMed/Web of Science list, ISSN no., date of publication, and HOD/Guide/Prof. In‑Charge certification.
